What is the Uniform Borrower Assistance Form (Form 710)?

It is the standard application Fannie Mae and Freddie Mac introduced for homeowners asking their mortgage servicer for help — such as a loan modification, forbearance, repayment plan, short sale or deed-in-lieu. It collects your loan and property details, income, expenses, assets and the reason for your hardship.

Is MyMortgageMod affiliated with Fannie Mae, Freddie Mac or my servicer?

No. MyMortgageMod is an independent tool that helps you fill in and print the form. We do not submit it, negotiate with your servicer or make decisions about your loan.

Where do I send the completed form?

To your mortgage servicer — the company you send your monthly payments to. Check your statement or call the servicer to ask for their loss-mitigation address, fax number or upload portal.

My servicer sent me its own version of the form. Which should I use?

Use whatever your servicer asks for. Many servicers have their own branded version; the questions are largely the same, so you can copy your answers from your MyMortgageMod application.

What else goes in the Borrower Response Package?

Usually the signed form, an IRS tax-transcript authorization form, income documentation (such as paystubs, benefit letters or tax returns) and documentation of your hardship. The wizard's review step lists what applies to you.

How is my information stored?

Your drafts are stored in the site's database, tied to your account, and Social Security numbers are masked on screen except the last four digits. Only you (and site administrators for support) can open your applications. You can delete an application at any time.

Does it cost anything?

No. And you should never pay an up-front fee to anyone promising a loan modification. HUD-approved housing counselors can help you for free.

Can a housing counselor help me with the form?

Yes. HUD-approved housing counseling agencies help homeowners prepare applications and talk to servicers, usually at no cost. Find one through the CFPB's housing counselor search or by calling HUD at 800-569-4287.
